Island Health's Community Health Services deliver in-home and community-based care across Vancouver Island, with no referral needed to access support. An intake clinician conducts a phone assessment to understand your health needs, determine eligibility, and identify the right services, including a home safety evaluation. Once referred, a health professional works with you to develop a personalized care plan, ensuring you have a say in all decisions about your care.

The service operates seven days a week, year-round, across three regional access centres covering all of Vancouver Island from Victoria to the North Island. Dedicated intake lines serve Victoria and the South Island, Nanaimo and the Central Island, and the North Island, with phone support available anytime to help older adults and their families understand what in-home support is available.

Island Health also offers specialized programs like Hospital at Home, which brings hospital-level acute care directly to patients' homes, and a caregiver support program to help family members and friends provide better care. Services are publicly funded and coordinated with your primary care provider to ensure seamless health care.
