Regional Geriatric Medical Outreach

Provides comprehensive geriatric assessment services to seniors in their own home. Patients may receive care from a team that may include a home visit from one or more of the following: Nurse practitioner Occupational therapist Social worker Pharmacist Geriatrician Recommendations may be provided to patients with the goal of improving their health and overall quality of life. Patient's family doctor will be given a report and an appointment with a geriatrician will be arranged if needed.

Geriatric Assessment Clinic / Seniors Health Clinic

Outpatient clinic serving older adults who live in the community. If the patient lives at home, in a retirement home or a long-term care facility and can come to the clinic, the patient will be seen by a geriatrician or a nurse practitioner who will complete the assessment, make recommendations and be in touch with the patient's family physician with the results of the visit. Orthopedic Rehabilitation

Outpatient Physiotherapy Services (Progression)

Hospital-based outpatient day program with a focus on rehabilitation. Patients are engaged in setting their treatment goals and are expected to participate in their assigned home program to help their full functional recovery. Virtual care is also available upon request.

Hospital-based program with a focus on rehabilitation. Rehabilitation is offered to adults with neurological, medical, orthopedic, and cognitive disabilities. Geriatric Psychiatry

Outpatient Program

Outpatient program integrating geriatric medicine and geriatric psychiatry anchored in primary care where the primary care provider remains the most responsible provider. Medical psychiatry team includes registered nurses, an occupational therapist, a geriatrician, a geriatric psychiatrist and a primary care provider. Under the supervision of geriatric specialists, RN Care Managers and allied health professionals will work with the patient and the primary care provider to manage both the mental and physical health needs with a collaborative care intervention for up to 16 weeks.
