Geriatric Assessment and Intervention Network (GAIN)

The GAIN Community Team supports frail older adults, (generally 65 years plus) adults living at home or in retirement, or, residents who are experiencing changes in support needs, safety concerns. GAIN teams provide specialized geriatric care to support frail seniors living at home or in retirement residences, who have multiple complex medical and social problems including:. Cognitive impairment Responsive behaviours Decreased function Falls or risk of falls Impaired mobility Incontinence and/or Multiple medications. The Durham Community Health Centre GAIN team consists of nurse practitioners, occupational therapist, social worker, pharmacist, dietitians, dedicated GAIN/Ontario Health atHo.
