Hospital Elder Life Program (HELP)
HELP is in alignment with the Delirium Aware Safer Healthcare (DASH) campaign that aims at reducing hospital-acquired delirium across Ontario. HELP is designed to prevent delirium by keeping hospitalized seniors oriented to their surroundings, meeting their needs for nutrition, fluids, and sleep, and keeping them mobile within the limitations of their physical condition. Hospitalized older patients may experience a decline in their physical and mental abilities, which makes it difficult for them to fully recover from their illness and return to their previous ability to function.

Step-Up Program (Progression)
Multidisciplinary outpatient program that provides physiotherapy, occupational therapy and/or speech language therapy for clients who have suffered a recent cerebral-vascular accident, acquired brain injury or other neurologically related impairment, and who have experienced a recent loss of strength or mobility. Beneficial for frail elderly and those recovering from a stroke or acquired brain injury, or neurological diseases including Parkinson's, brain tumour, Guillain-Barré Syndrome and Multiple Sclerosis. Clients are treated for two half days a week for a period of up to eight weeks.

Work-Fit Total Therapy Centre (Progression and Maintenance)
Physiotherapy clinic. Health professionals assess and treat conditions that affect the muscles, nerves, and bones that may be caused by work or sports or motor vehicle collision injuries, chronic conditions, or to recover after an operation (hip and knee surgery). Complex Transitional Care Department
Provides a transitional level of care when a patient no longer requires the hospital's acute care services. Services are provided by an interdisciplinary health team for people with medically complex, nursing, and therapeutic needs. Focus of the Complex Transitional Care (CTC) is to develop a plan of care that would enhance quality of life and facilitate discharge to a lighter level of care.

Colorectal Diagnostic Assessment Program
The program improves access to timely diagnostic testing and a coordinated approach to patient care supported by a multidisciplinary team of radiologists, surgeons and other key healthcare providers. Patient navigator helps coordinate and book all patient's appointments, and provide patients with support and education. Staff will inform the family doctor of the results and appointments, and keep them up-to-date about patient's care.
