Falls Prevention Program (Progression and Maintenance)

Interprofessional program that involves an in-home visit from an occupational therapist and pharmacist to help older adults reduce their risk of falls. Licensed therapists conduct in-home safety assessment and recommends assistive devices or home modifications. Pharmacists educate on proper use of medications and prescriptions.
