Perform in-home comprehensive geriatric assessment of age-related issues, and offer solutions to help seniors maximize their health, independence and quality of life. Geriatric Emergency Management
Consultation by a specialized geriatric nurse within a hospital emergency room to provide assessment, diagnosis, identification of "frail and at risk" elderly, and to initiate appropriate treatment in the hospital or community.

Healthy Living Clinic
Patients are referred to the clinic for assessment of a variety of concerns such as:. Recent changes in physical, mental or functional abilities; falls, impaired mobility, dementia, delirium, change in mood, decreased ability for self-care; major changes in support needs, caregiver stress, institutional placement being considered; and safety concerns – physical, psychological, social, and environmental; or a sudden increase in the use of health care services over the last 6 months. Hospital-based outpatient rehabilitation program for patients with conditions such as broken bones (fractures), or for patients after surgery. Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality.
How to access: Referred from Orthopedic surgeon from Humber Hospital * Rehabilitation must take place within 3 months of the surgery or fracture
